Seems many permits right now seem to not be spawning. Many have said Alioth isn't working and I gave up trying to get my Isinor permit as it no longer spawns in 3.0.3 that I can see (it was there in stations before the patch though)...
So I decided to go ahead and update the list of permits known so far in the game. There is quite a bit of out of date, incomplete or outright wrong information from various sources, so I decided to compile the best info I've been able to find so far. I've also found precious little information about how to actually get some of these permits (or if it's even possible at all). So if you know something, share it and I will update it. Please contribute additional permit locations, and the best places to obtain them, as well as any other relevant information (such as why you'd want to get the permit or where the ideal locations are, if any).

Important things to keep in mind when obtaining permits
The controlling faction is not always the permit providing faction.
You can rep up at any place the faction is at, but only systems within 15 light years of the permit faction's home system will provide the permit.
NEWS/UPDATES:
14 August 2017: I have decided to discontinue updating the rep systems by hand and instead will provide a direct link to eddb for each faction, which did not exist at the time when I originally created this thread. Credit to Factabulous on page 57 for finally for making this suggestion.
Naturally, there are still permit-locked systems out there that may yet be discovered, while there are others that should be taken off this list. I will check back every so often to see if there are any new updates. Thank you so much to people who have contributed to this thread!

Tips on getting allied quickly with an Independent Permit faction
Exploration
5-7 million credits dumped at a station OWNED by a permit granting faction gets you to allied quickly. So take that trip to Sag A*, come back, hit up a few hundred neutron stars, and get allied. While the exploration takes forever, getting allied will go by quickly once you get back. Just be sure to re-outfit your ship with better shields, armor and thrusters and chaff to withstand the odd NPC interdiction.
Trade
Trade is also an excellant way to get to allied status while making some decent money. Here are some trade routes that will help you get allied.

* Several stars (just a small handful) in the Bernard's Loop Area that are NOT in the Col 70 or Horsehead Dark Region may also be permit locked. If you find any, please report it. But Bernard's Loop itself (Trapezium Sector tagged stars specifically) does not appear to have any locked systems. Similarly, not every Col 70 or Horsehead Dark Region star is locked, just a very large percentage of them. If you leave the Bernard's Loop area, you'll be more likely to find Col 70 Or Horsehead Dark region stars that are not locked.
this is all user error...somehow...fdev/ed are flawless!!!
anyway, ThePunisher and others put in tickets and harassed fdev when they found out how fast others where ranking up, demanded it be addressed NOW NOW NOW, because, somehow it hurts their pride, having worked hard in the past to get something that became easier to get down the line... as such, fdev did the fdev thing, and, addressed it by pulling those missions, with the intent of moving them to follow-on status and slowing the rank gain drastically, hopefully to vet era levels so the vets can feel pride again...and not need to have them removed for re-balance again...
Except it's not an easy win button, the change was simply to surface missions THAT WERE ALREADY THERE to their own section on the missions board, to make them easier to find. Except that this is simply not working.
It's got absolutely zero to do with the speed people can rank up or get permits, and I don't see why you're trying to connect the two issues.
they removed the missions to address the complaints from "Vets" that people where ranking up to fast, the plan is it will show them as follow on's but, for now, as with all other issues fdev addresses, they just disabled it first....makes sense, a week of nobody gaining ranks should help make "Vets" feel better, salv their wounded pride(ThePunisher actually put in tickets as well as complaining on forums because he was so upset people where ranking to fast.
to get your permit you must rank up, that can nolonger be done PERIOD those missions are not in game, and will not be in game till next patch... because people where ranking to fast and it offended vets, the same way they are now asking skimmer missions be removed again because people figured out how to have a person in an srv trigger them and ships blow them up... i expect they will remove them again soon.. along with anything else people find ways to exploit or make "too much money/rank/etc" from..
they are balancing the game by blocking rank access till they can fix it properly... the only people complaining are those who want an instagib/instant win button!!!!
FDEV did NOT remove the rank up missions to address complaints. That's simply untrue, not backed up by any facts, and contradicts the 3.0.3 changelog. I've not idea where you're pulling this idea from, but it's simply wrong. FDEV DID NOT REMOVE THESE MISSIONS ON PURPOSE.

Until you actually back up anything you say with some kind of proof or reference from FDEV I'm not inclined to pay any further attention to you.
If you check the bug forums, FDEV have acknowledged there is a bug with these missions spawning. 3.0.3 introduced a change to these mission types so that they are supposed to appear at the top of the list like a chained mission. This isn't working, and the missions actually aren't spawning at all.
Permit missions are spawning under chained missions for me, as per 3.03 design. It is important however, that the current system is closer than 15 ly to the permit requiring system (Info from further up the threat). I had issues with the Pi Mensae permit until I tried a system close to Pi Mensae.

Now that the mission board doesn't fail and state that it can't retrieve all the missions, I was finally able to pick up the Isinor permit. So seems like it's been fixed now...
I don't seem able to get the Nastrond permit mission to spawn. I'm allied with Defence Party of Nastrond and have been board hopping at multiple stations in Wonorne Nu and LP 295-2, both of which are within 15ly of Nastrond. This is the last permit I need that's not locked behind any rank. RNGeezus enjoys seeing me suffer.
That's actually been like that for awhile, and you can't get MBOONI permit as that system has no population so there is no faction you can Ally with to get the permit. And Kamba is not Permit locked anymore.

DRYIO FLYUAE IC-B C1-377. It was found by a CMDR on Ps4 I think. A security permit is required. It's literally in the middle of no where. Not sure if it's relevant to this thread but I posted it in the alien/mysteries thread and I was advised to report it here as well. Again not my find and I don't know the name of the CMDR that found it but it's certainly a strange one.
